Description
Ensure that people with or at high risk of diabetes have access to community resources and support to prevent, delay or manage chronic conditions once they occur. Classes include Living Well with Diabetes and National Diabetes Prevention Program (NDPP). Stepping On reduces the risks of falls in older adults. Tai Chi is available for older adults.

Description
The Baby Your Baby Card covers outpatient pregnancy related services such as doctor's appointments and prescriptions. It does not cover labor and delivery.
