Description

The Children's Health Insurance Program, or CHIP, is a state health insurance plan for children. Depending on income and family size, working Utah families who do not have other health insurance may qualify for CHIP.

The Utah Horizon - Electronic Payment System

Description

The goal of the financial assistance programs is to increase family and individual income by focusing on employment, and where appropriate, child support and disability payments. Cash assistance combined with supportive services and personalized case management is meant to help eligible families and individuals achieve economic self-sufficiency and enhance the well-being of children. Must apply and qualify for a DWS SNAP or financial assistance program to receive EBT Card. If a client's case closes, it is advised that they keep their EBT card to avoid reapplying for help in the future. For lost or stolen EBT cards, questions or to check balance on the card, call the Horizon Card Helpdesk. Customers can also access their EBT account through the ConnectEBT Mobile App which can be downloaded from the AppStore or Google Play. Residential Eviction Protection

Description

A program providing free mediation assistance to help resolve conflicts between landlords and tenants related to eviction attempts during COVID-19.

Documentation proof that rent was current as of March 31, 2020 and ONE of the following: job or wages lost due to COVID-19; subjected to mandatory quarantine order by the Utah Department of Health or local health department; tested positive for COVID-19.

Any renter who was current with rent as of March 31, 2020, tested positive for COVID-19 or suffered loss of job or wages due to COVID-19 or was subjected to mandatory quarantine by a department of health.
