Fees

Sliding fee based on income; all insurances types accepted.

Intake Procedure

Call for appointment if established patient (within the last 3 years); walk-ins accepted. New patients are advised to walk-in and register before noon M - F. Pharmacy, Dental, and Behavioral Health services are available for established patients only. COVID-19 UPDATE: In-person visits are to check-in through the front of clinic and complete screening questionnaires. Those that are symptomatic or high risk may be rescheduled.
