Private, nonprofit, non-residential facility that teaches independent living skills to people with disabilities. Description
Opportunities are provided for students 13 to 22 years of age, with disabilities to participate in recreational and leisure activities in the community. Trips are organized to community activities and events in order for persons with disabilities to interact with people in the community while learning how to use and access community resources. Description
Nursing Home Transition assistance is for persons that do not want to go to a nursing home or want to transition out of a nursing home. Assistance is provided by way of connecting those with disabilities, to the resources in the community to transition out of a care facility/nursing home or help find supports to stay in the community if needed.
