These services include CHIP programs, EBT Card Services, Food Stamps/SNAP Applications, general relief applications, Medicaid applications, TANF applications, and non-Emergency medical transportation. Also offers different expense/ payment assistance that include burial/ cremation, Childcare, mortgage, rent, refugee/ entrant cash assistance, and utility service payments.

Description:

Description:

The rent payment program is part of the Emergency Assistance Homeless Prevention Program (EA) that provides immediate short term help for destitute families to remedy or prevent homelessness due to eviction.

Eligibility:
Family must be facing eviction because of past due payments that resulted from a crisis beyond their control. Must have a dependent child, or a woman pregnant in her third trimester in household and demonstrate the ability to make future payments. The household must meet income and asset requirements and have exhausted all other resources.

Description:

Medicaid may provide reimbursement for non-emergency medical transportation, if currently eligible for Traditional Medicaid services and do not have transportation to get medical care. Transportation services include: UTA Transit Card, FlexTrans, Dial-A-Ride, ModivCare or personal mileage reimbursement.

Description:

Refugee cash assistance (RCA) is provided only to newly arriving refugees, new asylees, certified victims of human trafficking, and other persons eligible for refugee services within the first 12 months of arrival or certification.

Eligibility:
To qualify, client must have a current immigration status identifying them as a refugee or asylee, and have arrived in the United States no later than eight months prior. Clients must also fall below specific income thresholds.

Description:

The Children's Health insurance Program (CHIP) covers children who are not covered by other health insurance and do not qualify for Medicaid.

Eligibility:
Based on household size and income. Children must be under age 19, U.S. citizens or legal residents, and not covered by other health insurance. There are no asset limits.

CHIP has quarterly premiums of $30 or $75 as well as co-pays.

Description:

Provides time limited cash assistance and case management services to single adults and married couples that have no dependent children residing with them 50% or more of the time.

Eligibility:
Must verify a physical or mental health impairment that prevents basic work activities in any occupation for at least 60 days or longer from the date of application. Also, must also meet certain citizenship, household (no dependent children in their care) and income requirements.

Temporary cash assistance program for families. While in program individuals will learn to gain greater financial independence, learn work skills that may lead to better paying jobs, learn valuable skills, and be positive role model. Program can provide up to 36 months of financial support to a family within a lifetime.
