Community clinic providing medical, dental, prenatal, general counseling, and navigator programs. Serves all residents regardless of ability to pay, and offers specialized services to meet the needs of migrant and seasonal agricultural workers and their families through Utah Farm Worker Program (UFWH). Also refers patients to other existing health care providers and resources.

General Dentistry
Description:

Offers preventative and restorative dental care, dental hygiene, and dental care education.

Appointment preferred; walk ins will be accommodated when possible.

Description:

Behavior specialist screens for depression, alcohol and drug use, and if needed, makes referrals to mental health services.

All ages; serves all residents regardless of ability to pay.

Navigator Programs
Description:

Take Care Utah navigators/certified application counselors are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP.

Provides medical, dental, mental health, health education, and outreach service to all residents regardless of ability to pay. Services include depression screening, alcohol and drug screening, laboratory testing, radiology, pharmacy, chronic illness management, patient education, and referral services to other medical and mental health resources.

Description:

Provides prenatal and delivery services as well as ob/gyn.

All expectant women. Clinic works congruently with specialist for high risk pregnancies.
