Provides a variety of services to the growing population of senior/aging citizens throughout the county including: senior activity centers; case management of health-related needs; home delivered meals to eligible seniors; in-home respite care; household chore, shopping, transportation and errand assistance; and long-term care ombudsman programs. Related volunteer opportunities available.

Description:

Long-term ombudsman volunteers advocate for residents ages 60 and older who live in long-term care facilities by monitoring care and treatment. Ombudsman volunteers conduct routine facility visits to interview residents about their care and complaints.

Description:

Information on available services and assistance connecting with community resources to help with the care of a loved one and ease the role of caregiving; advice and guidance about options and methods for dealing with caregiving responsibilities; and financial help for some respite care services and some equipment and devices.

Eligibility:
Caregivers may be any age although they must be experiencing a significant amount of stress related to caregiving responsibilities. The individuals cared for must be 60 years of age or older and experiencing problems maintaining day-to-day tasks such as dressing, bathing themselves, toileting, transferring, homemaking, etc.

Description:

Volunteers use their time and experience to help people with Medicare and Medicaid avoid falling for scams. They work one-on-one with beneficiaries, give presentations to groups, and represent the SMP at events. Training is provided. In other opportunities, volunteers work with the Chronic Disease Self-Management Program (CDSMP), known in Utah as Living Well with Chronic Conditions (LWCC). It is an evidence-based program developed by Stanford University's Patient Education Research Center. It is a 6-week workshop that takes place once a week for 2 hours, in groups of 10-15 individuals.

A voucher program for seniors and persons with disabilities. Vouchers allow clients to arrange rides to medical appointments with a driver of their choice (friends, family, neighbors) who are then reimbursed for the mileage by UTA.

Food serving: Volunteers are needed to assist with serving the lunch meal provided at Senior Citizen Centers. Meals are provided M - F. Food delivery: Meals on Wheels (MOW) volunteers deliver meals to home bound individuals and help them remain living independently in their home. Each shift runs M - F 10:30 am - 1 pm, with flexible schedule.

Description:

Meals On Wheels Program helps older people remain independent in their homes by providing one or more nutritious meals per day to eligible individuals unable to manage their own meal preparation. Special diets for health reasons, prescribed by a physician, can also be provided.

Eligibility:
The degree and nature of an illness, disability, or isolation determines if homebound or take-out meals are needed, and how often. Must be Davis County resident, 60+, homebound (unable to leave home without assistance of others), and does not drive a vehicle.
