Options for Independence - Logan

A non residential center for independent living designed for people of all ages and disabilities. Provides services to individuals with disabilities to facilitate their full participation in the community and raise the understanding of disability issues and access to the community. Main services and additional support are provided depending on the needs of the individual with the disability. Services included, assistive technology equipment loans, health and disability related support groups and social clubs, peer to peer networking, disability rights groups, in home developmental disabilities habilitation programs, ADA implementation assistance, information and referral.

Assistive Technology Equipment Loan
Description:

Provides and maintains assistive devices for the general public with priority to residents in the program service area. The loan of equipment is up to 90 days; possible extensions if necessary.

Eligibility:
All people of all ages who experience a physical, sensory, cognitive, or mental disability. Eligibility for some opportunities is based on economic need.

M - Th 7 am - 4 pm; F 7 am - 1 pm

Health/Disability Related Social Clubs
Description:

Provides social, recreational opportunities, and group activities for people with visual, physical, and mental disabilities to gain more control and independence over their lives.

Eligibility:
People of all ages who have a physical, sensory, cognitive, or mental disability are invited to participate in Center activities. Eligibility for some services is based on economic need, however, most independent living programs and services are available to all people with disabilities at no cost.

Independent Living Skills Instruction
Description:

A nonresidential independent living center where people with visual, physical, and mental disabilities can learn skills to gain more control and independence over their lives.

Eligibility:
People of all ages with physical, sensory, cognitive, or mental disability are invited to participate in the Center's activities. Eligibility for some services is based on economic need, however, most independent living programs and services are available to all people with disabilities at no cost.

Health/Disability Related Support Groups
Description:

Provides support group interaction for people with visual, physical, and mental disabilities and their family members. Groups include health relationships, low vision, Wellness Recovery Action Plan for healthy aging/healthy life, woman's anxiety, therapy, mental well being, and youth.

Disability Rights Groups
Description:

Complies with Title VI of the Civil Rights Act of 1964 and all related regulations and relevant guidance. Assures that no person in the United States shall, on the grounds of race, color, citizenship status, national origin, ancestry, gender, sexual orientation, gender identity, gender expression, transgender status, age, religion, creed, physical or mental disability, marital status, veteran status, political affiliation, or any other factor protected by law be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving federal financial assistance. Description:

Casework to ensure clients with visual, physical, and mental disabilities receive the help needed; case plans are targeted.

Description:

A nonresidential independent living, and transitional youth and employment readiness center for people with visual, physical, and mental disabilities, to learn skills to gain more control and independence over their lives.

Peer to Peer Networking
Description:

Available for people with similar disabilities who may have difficulty handling specific problems related to their disability and want to help others. Support and training on community resources through the guidance of those who have personal experience. Available individually or in a group setting as required.

Description:
