Application form. Clients may be referred by another Social Service Agency (Workforce Services, church charity organization, etc.), or must provide income verification, i.e., pay stubs, tax returns, etc for everyone in household over the age of 18 (not applicable to the Commodities Supplemental Food Program for Seniors). Also need social security cards for everyone in the household, picture ID for the head of the household, proof of residence, food stamps printout (if applicable). Document must be updated with the food bank every six months.
Eligibility
Clients who receive emergency food are required to apply for food stamps.

Eligibility
Eligible families must have a dependent child in the home. Generally, if you are already receiving food stamps, Medicaid or are enrolled in the TANF program and working with a case manager you may be eligible for this assistance.
