Residents of the of Box Elder, Cache, and Rich counties may apply for treatment at a subsidized rate.

Description:

Provides low-income women, infants, and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding woman, woman who has just had a baby, or child less than 5 years of age who meet income requirements.
