Utah County Health Department - Payson

Provides services that promote the achievement and maintenance of optimal health for all people in the community. Services include birth certificates, international travel advisories, WIC, immunizations, and death certificates.

Description:

Copies of death certificates are available beginning 1905. Prior to August 2006, only Utah County records are available. Death records for other Utah counties prior to August 2006 can be obtained at the office for an additional fee. Death certificates can only be filed for deaths occurring in Utah County.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

Program provides low-income women, infants, and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding women, women who have just had a baby, or child less than five years of age who meet income requirements.

Provides travel immunizations and education to those traveling outside of the United States. Plan to be with the immunization staff for up to 2 hours.

Eligibility:
Yellow Fever Vaccine, Cholera vaccine and Rabies vaccine are not offered in the Payson Clinic

A fee of $25 is charged for travel counseling without vaccine services. Vaccine costs vary.
