RISE Services - Logan

Provides home and community based support services for children and adults with developmental and other disabilities, children with mental health challenges, and aging adults. Services include home respite care, residential care, behavioral therapy, employment programs, and supported living services.

Description:

Behavior specialists identify and evaluate the causes of problem behaviors for individuals who may have a range of developmental disabilities, backgrounds of abuse and neglect, or mental health diagnoses. Specialists then work with the individual, the family, and other natural supports, to develop strategies and to prevent, minimize, or eliminate problem behaviors. Specialists also develop and recommend interventions, teach skills, and promote independence, to help the individual be successful in their homes and community.

Eligibility:
Children and adults with a documented diagnosis of developmental and other disabilities, children diagnosed with mental health challenges, and aging adults.

Description:

Adults with disabilities receive care that is specific to their needs in a group home setting. Residents are encouraged to make individual choices in their daily routines, their roommates, staff, and the goals they would like to achieve. Offers up to 24 hours of personal care for adults with disabilities who can live with some independence.

Includes meeting fundamental needs such as housekeeping, personal care, medical assistance, grocery shopping, and budgeting skills.

Provides direct support professionals, to assist with temporary care for children and adults. Direct care is tailored to each individual, based on individual needs. Provides a safe place for the child or adult, and maintains consistency and continuity of care.
