International Travel Advisories
Description:

Provides travel health information and administers some travel vaccinations. Makes referrals to partner clinics for vaccines not available.

Death Certificates
Description:

Provides copies of, and allows changes to, birth certificates in Carbon, Emery, and Grant counties.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

M - Th 8 am - 12 noon & 1 - 5 pm
Intake Procedure:
Requests can be made online, in-person, and through the mail.
Fees:
$30 for initial copy, $10 for each additional copy.

Description:

Provides permits for both permanent and temporary food service establishments. Also refers out to verified food handler courses.

Description:

Breast Examinations
Description:

Provides breast examinations to qualifying women in order to detect cancer.

Age 40-64, income below 250% poverty level.

Prenatal Care
Description:

Financial assistance through the Baby Your Baby temporary Medicaid program. The card can cover outpatient pregnancy related services until a full medicaid application can be processed.

Eligibility:
Must be a U.S. citizen or have a green card for 5 years. Must also meet income requirements, see website for guidelines table.

WIC
Description:

Program provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Pregnant or breastfeeding women, women who have just had a baby, or children less than five years of age. Must also meet income requirements.

Prenatal/Postnatal Home Visitation Programs
Description:

Home visiting program that provides education and empowerment to prenatal families and families with infants and children birth to 5. Visits are conducted by registered nurses.

Eligibility:
People who qualify for Medicaid or WIC, first time or single parents, and parents or children with physical or mental disabilities.

Description:
