Utah Insulin Savings Program

A program that allows Utahns who are diagnosed with Type I, II, or gestational diabetes, and using insulin for treatment, to purchase insulin at discounted rates. Program is primarily designed to help individuals who do not have health insurance or are covered by insurance that requires a large deductible.

A program that allows Utahns who are diagnosed with Type I, II, or gestational diabetes and using insulin for treatment, to purchase specific brands of insulin at discounted rates. Program is primarily designed to help individuals who do not have health insurance or are covered by insurance that require a large deductible. Brands are Lantus, Toujeo, Novolin, Novolog, and other Novo brand mixed insulins.

Eligibility:
Must be a legal resident of Utah, been diagnosed with diabetes, use insulin to treat diabetes, not be an employee nor dependent of an employee of the State of Utah or PEHP, where there are covered insulin related benefits.

Intake Procedure:
Complete online application. Once reviewed and verified, ID card and information will be sent via email, which would then be presented to the individual's pharmacist in order to receive the discounted rate of the program. Call for additional information.
Fees:
No cost for the discount card.
