Description:
Provides an online voucher for eyeglasses. Vouchers typically cover only the cost of a basic pair of single or lined bifocal eyeglasses.
Eligibility:
To be eligible for the program, applicants must: Meet the U.S. Poverty guidelines and have had a recent eye exam within the last 12 months. Must have no other resources available to pay for glasses, including federal or state programs or assistance from local charitable organizations.
Service Hours:
M - Th 9 am - 12 noon & 1 - 2 pm
Intake Procedure:
New Eyes accepts applications from both individuals and referral agencies. This can be a social service agency, a primary care physician (not an eye doctor), a medical clinic, or a house of worship. Online application only.
