United Healthcare Children's Foundation

Provides medical grants to help with expenses for children and expenses for health related services not covered or full covered by their health insurance plan. Children must be 16 years old or younger while meeting other grant requirements.

Medical Care Expense Assistance
Description:

Families may receive up to $5,000 annually per child ($10,000 lifetime maximum per child), and do not need to have insurance through UnitedHealthcare to be eligible.

Eligibility:
Parents of children receiving serious and financially-challenging health care treatment. Children must be 16 or younger at the time of application, have a Social Security Number issued by the Social Security Administration (TIN numbers are not accepted), must be covered by commercial/private insurance, and under license medical professional. Family must not exceed maximum eligible family income as documented on IRS Tax Form 1040.
