Helps to subsidize monthly health insurance premiums for those enrolled under their employer or COBRA. Can cover up to $300 per adult per month.
Eligibility:
Adults 19-64 who are legal residents or citizens and not covered by health insurance. The cost of the least expensive health insurance should be more than 5% of total income.
Service Hours:
M - F 8 am - 5 pm
Intake Procedure:
Apply online or call for a paper application. Apply before the date your health insurance begins.
