Description:

Eligibility:
Home Health Plus through CNS serves infants, children, new mothers and adults with home health care needs with a physician's order.

Home hospice care available to home-bound clients who are nearing the end of life. Hospice care is meant to enhance the quality of a person's end-of-life journey.

Home bound with a physician's order; people whose life expectancy is 6 months or less due to a life-limiting medical diagnosis.
