Medication Assisted Maintenance Treatment for Substance Use Disorders
Description:

Offers outpatient treatment for opioid addiction. Suboxone may be included in treatment.

Eligibility:
18 years and older, dependent on opioids for at least one year, and reside within 50 miles of Layton, Orem, Salt Lake, and Taylorsville.
Service Hours:
M - F 5:30 - 11 am
Intake Procedure:
Must arrive for intake in a state of withdrawal of at least 24 hours (unless pregnant), complete paperwork, a physical examination, drug screening, TB test (or proof of test within past two years), and pregnancy test for females.

Substance Use Disorder Counseling
Description:

Offers outpatient substance use counseling and referral.

Eligibility:
18 years and older, dependent on opioids for at least one year, and reside within 50 miles of Layton, Orem, Salt Lake, and Taylorsville.

Substance Use Disorder Education/Prevention
Description:

Offers substance abuse education that promotes holistic recovery.

Eligibility:
18 years and older, dependent on opioids for at least one year, and reside within 50 miles of Layton, Orem, Salt Lake, and Taylorsville.
Service Hours:
M - F 5:30 am - 2 pm
Intake Procedure:
Must arrive for intake in a state of withdrawal of at least 24 hours (unless pregnant), complete paperwork, a physical examination, drug screening, TB test (or proof of test within past two years), and pregnancy test for females.
Fees:
Intake fee is due at intake appointment via cash, money order, or credit card issued in applicants name. Treatment costs and payment schedules will be discussed during the intake process. Payment for treatment is due in advance in most cases.
