Provides Support Coordinators who monitor the health and safety of those with qualifying disabilities. Coordinators also inform persons about available services, and help with the selection of providers whom offer those services and monitor through completion.

Includes heavy household chores such as washing floors, windows, and walls, snow removal, lawn care, tacking down loose rugs and tiles, moving heavy items of furniture in order to provide a safe living environment.
