Summit County Health Department - Coalville

Provides early identification and development services for families of infants and toddlers with developmental delays or disabilities, ages birth to three. These services are provided through the coordinated effort of parents, community agencies, and a variety of professionals. Most services are provided in the family home or natural environment and community settings such as child care.

Birth control options counseling, and purchase of birth control pills, Depo-Provera shot, patch, or Nuva ring.

Provides uninsured and underinsured women with pregnancy testing.

Provides breast and cervical cancer screenings and education to women who qualify.

The WIC Program provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

The health department offers confidential STD testing and treatment. Rapid HIV, Hepatitis B/C, Syphilis, Chlamydia/Gonorrhea.

Immunizations for children, adolescents, and adults.

Copies of death certificates for deaths in Utah are available beginning 2000. Death Certificates prior to 2000 can be made available through a search.

Provides services that promote the achievement and maintenance of optimal health for all people in the community.

Provides copies of birth certificates for those born in the state of Utah. Birth years 1905 to present can be requested in the office.

The food safety program works to ensure safe food service practices. Class are taught throughout the year. Online classes must be taken through approved vendors.
