Give Kids a Smile is an annual dental event that provides low-income children without dental insurance dental care. Contact Community Health Connect for more information.
Eligibility:
Children 5-12 years old without dental insurance. Children older than 12 will be placed on a wait list. Families need to be at or below 150% of the federal poverty level.
Service Hours:
Office: M, W 9 am - 6 pm; Tu, Th 9 am - 4 pm Event: 3rd Sa in February
Intake Procedure:
Must pre register by emailing or calling Sarah ([email protected]) by the last week of January. Parents need to explain the situation and the number of children that need to be seen. Fill out and return health history consent form before the last week of January.
