Provides general mental health counseling along with long term support to minimize confusion, enhance individual choice, and support informed decision-making in accessing long term support services in the community.

Assists people with disabilities who are living in nursing homes or other types of institutions to relocate into a community-based living environment. Also assists with technology use and with signing up for section 8 housing.

Consumer Advocacy involves center staff working with individuals to obtain necessary support service from other community agencies.
Community Systems Change Advocacy involves staff, board members and volunteers initiating activities to advocate social and political change for individuals with disabilities.

Offers work incentive counseling, skills testing, resume writing, job search and placement assistance, and post-employment services.

Provides temporary durable medical equipment for individuals with disabilities until permanent equipment can be accessed.

Provides students with disabilities ages 14-21 opportunities to explore career interests, improve workplace skills, and obtain work experience. 13-week development class is required for all participants.

Offers training and classes to increase self-reliance. Topics include money management, basic computer skills, cooking, communication skills, and using public transportation, also youth and adult activities.

Connects individuals to another person who has a similar disability and has experienced similar barriers in living independently to provide peer support.

Assists people with disabilities who are living in nursing homes or other types of institutions to relocate into a community-based living environment.

Connects individuals with disability-related community resources.

Offers evaluation for the need of equipment for consumers. Also assists with the process of obtaining resources for the purchase or repair of equipment. Equipment can include mobility aids, hearing aids or home modifications.
