Community Health Connect

Provides access to medical, dental, optical, and mental health care for the low-income and uninsured. Services are not provided on site. Referrals are made to local providers for specialized medical needs.

Description:

Provides referrals for low-income, uninsured individuals and families living in Utah County to receive mental health services.

Eligibility:
Must reside in Utah County, be at or below 150% of the federal poverty level, and be uninsured. For both children and adults.
Service Hours:
M, W 9 am - 6 pm; Tu, Th 9 am - 4 pm
Intake Procedure:
Fill out application online or in office.
Fees:
$20 six-month membership per individual. Cash only.

Description:

Connects low-income families and individuals with donated dental services including, but not limited to, root canals, orthodonture, and crowns.

Eligibility:
Must reside in Utah County and be at or below 150% of the federal poverty level. Services for those 12 and older.
Service Hours:
M, W 9 am - 6 pm; Tu, Th 9 am - 4 pm
Intake Procedure:
Fill out an application online or in office. A 30 minute oral health education class is required before a person can be seen by a dentist. Classes are held on the second Wednesday of each month.
Fees:
$20 six month membership per individual. Cash only.

Eligibility:
Must reside in Utah County and be at or below 150% of the federal poverty level. A referral from a primary care provider from Mountainlands Community Health Center in Provo is required for specialized health referrals. Serves all ages.
