General Health Insurance Information/Counseling
Description:

Certified Application Counselors on-site to assist with applications for Medicaid, CHIP, and the new Health Insurance Marketplace.

Eligibility:

M - Th 8 am - 4:30 pm; F 8 am - 3 pm; Appointments available up to 1 hour before closing.
Intake Procedure:
Walk in or call to schedule an appointment with a Certified Application Counselor.

Navigator Programs
Description:

Take Care Utah navigators/certified application counselors are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP.

Description:

Certified Application Counselors on-site to assist with applications for social service programs like SSI, the HEAT Program, DWS Eligibility, SNAP, and prescription discounts.
