Care coordination via phone, text, and email to help families navigate resources. Follow-up is conducted to ensure that families successfully connect to services. Information on general child development and parenting topics and referrals to community resources, such as early intervention agencies, family therapy organizations, literacy support organizations, mentoring and tutoring programs, parenting classes and support groups, and recreational opportunities.
Provides developmental questionnaires for children ages 0-6 and an emotional well-being screening for parents, prenatally and postpartum.Once the screening is submitted a member of the Help Me Grow team will call the family to discuss the results and provide additional resources as needed.
