Provides case plans for the evaluation, treatment and/or care of individuals who have mental, emotional or social problems and need assistance in arranging for services.
Eligibility:
Serves patients with Medicaid. Does not accept PCN, CHIP or private insurance. Services and funding may be available for eligible uninsured individuals. Call for more information.

M - Th 8 am - 6 pm; F 8 am - 5 pm; Walk in intake Th 8 am - 1 pm; Scheduled intakes as needed, call for appointment
Intake Procedure:
New clients must complete a Mental Health Assessment during a Walk in Intake. Walk in intakes are on a first come first served basis, while space is available. Call for a scheduled intake if necessary. Children must be accompanied by legal guardian.
Fees:
Traditional and non-traditional medicaid services are covered at 100%. Does not accept PCN, CHIP or private insurance. Funding may be available for eligible uninsured individuals . Call for more information.
