Prenatal Care
Description:

The Baby Your Baby Card covers outpatient pregnancy related services such as doctor's appointments and prescriptions. It does not cover labor and delivery.

Eligibility:
Financial help is available on a temporary basis to Utah women who are pregnant and do not have the money to pay for prenatal care. Income restrictions apply. US Citizens or Green Card holders for the last 5 years may apply.

Child Passenger Safety Seat Inspections
Description:

Car seat safety check to make sure your car seat is the right size for your child and that it's properly installed in your vehicle. Vehicle and child must both be present.

Description:

Provides birth and death records and certificates for Utah. Older records may be received by contacting the Utah Department of Health

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

Description:

Provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Pregnant or breastfeeding woman, woman who has just had a baby, or child less than five years of age who meet income requirements.
