Hospice services include homemaker assistance, required medical equipment, medications, medical care supplies, therapy (physical, speech, occupational), counseling, legal document assistance (wills, powers of attorney, and funeral planning).
Eligibility:
Adults over 18 with a life limiting illness with an estimated life expectancy of 6 months or less.
