University Developmental Assessment Clinics

Medical center that evaluates children with or at risk for developmental delays throughout Utah. Provides developmental assessments, neonatal care, speech and language evaluations, and infant and early childhood mental health care.

Speech and Language Evaluations
Description:

Provides assessment and referral for children with speech, language, or hearing developmental disabilities.

Eligibility:
Referral required. Further requirements depend on the specific clinic, contact for details.
Service Hours:
M - F 8 am - 4:30 pm
Intake Procedure:
After someone besides the parent completes the online referral form, call or visit website to schedule first appointment.

Neonatal Care
Description:

Provides follow up care and evaluation for children who have spent time in a NICU. Provides a variety of assessments at intervals from 3 months to 4 years of life. Visits available either virtually or in-person.

Eligibility:
Referral required. Children who have been in a NICU and fall under one of the following: require ECMO, are diagnosed with HIE, have a gestational age less than or equal to 26 weeks, or have a birth weight less than 1250 grams.
Service Hours:
M - F 8 am - 4:30 pm
Intake Procedure:
Referral from a NICU in the state of Utah or a medical care provider.
Fees:
Varies based on insurance and services provided. Accepts most private insurance and medicaid. No cost for uninsured individuals.
