Substance Use Disorder Counseling
Description:

Treatment is evidenced based and cognitive behavioral in a format emphasizing personal accountability and skills development to support drug free living.

Eligibility:
Clients may be referred from court systems, AP&P, DCFS, DWS, or may be voluntarily self-referred; High-risk (pregnant women and IV drug users) have admission priority.

Adolescent/Youth Counseling
Description:

Outpatient treatment to children and their families eligible for Medicaid. Interventions include individual therapy, group therapy, family therapy, and medication management services. Treatment works to decrease adolescent substance abuse, reduce problem behaviors.

Eligibility:
Children and youth between the ages of 0 through 18 who are eligible for Medicaid, and who reside in Weber and Morgan Counties.

General Assessment for Substance Use Disorders
Description:

Psychiatric Medication Services
Description:

Medication management services that may be provided to individuals whose psychiatric needs include administrating, reviewing, and monitoring medication and/or medication regime. Also includes medication assisted maintenance treatment (MAT) for opioid use disorder.

Eligibility:
Youth or adults who are eligible for Medicaid, who reside in Weber and Morgan Counties. Weber Human Services also provides crisis services for all residents of Weber and Morgan Counties, and services for individuals who are civilly committed.

Psychiatric Case Management
Description:

Psychiatric support can be offered when necessary as part of a rehabilitative treatment plan. Case management assists eligible clients not only in gaining access to needed medical, social, educational and other services. Also ensures coordination between all agencies and providers involved in a client's treatment.

Weber and Morgan County residents who are eligible for Medicaid. There are also limited services available for individuals who have no insurance or other payment sources.

Description:

Independent Living Skills Instruction
Description:

Provided in an outpatient, day treatment program, residential program, or other appropriate setting. Assist individuals in developing competence in basic living skills such as meal planning, shopping, food preparation, money management, mobility, grooming, personal hygiene, and maintenance of a living environment.

Adult individuals who are eligible for Medicaid, who reside in Weber and Morgan Counties.

Children's In Home Respite Care
Description:

Respite care and family support - gives families of children with at-risk behaviors a break from the demands of raising a family. Short term in-home and out-of-home services are available.

Families with children between the ages of 0 through 18 who are eligible for Medicaid, and who reside in Weber and Morgan Counties

Adult Residential Treatment Facilities
Description:

A men's and women's combined residential facility for sixteen 'Seriously and Persistently Mentally Ill SPMI' clients (9 male and 7 female) and 1 placement bed available for a client in transitional or hospital diversion/crisis situations. Also coordinates housing placements in various privately held rental units.

Individuals eligible for Medicaid, who reside in Weber and Morgan Counties.

General Counseling Services
Description:

Adult mental health and substance abuse outpatient and residential services offered to individuals, families and groups. Outpatient services include individual and group behavior management, individual and group therapy, and medication management.

Individuals eligible for Medicaid, who reside in Weber and Morgan Counties; limited services available for individuals who have no insurance or other payment sources based on funding and availability.

Substance Use Disorder Education/Prevention
Description:

Comprehensive prevention plan to address local prevention needs. Works with local community coalitions in analyzing information from the Student Health and Risk Prevention Survey (SHARP) to help direct services and leverage resources to best meet the needs of the community. Offers presentations in schools and other community locations, and a variety of parenting and life skills courses around the community for youth and adults.

Parenting Skills Classes
Description:

Provides a wide variety of parenting classes (at various locations) for both parents and youth, including: Guiding Good Choices,
Strengthening Families, Love and Logic, emotional coaching, anti-bullying, and stress management.

Classes available to parents and caregivers of children of all ages; some classes available for youth themselves.

General Addictions/Substance Use Disorder Support Groups
Description:

Various support offered as part of a rehabilitative treatment plan. Also offers support and education for children with a friend or family member struggling with addiction. Children's classes run for 8 weeks; attendees can join at any time

Eligibility:
For children's group: age 4-18, with alcohol/drug addicted parents and their caregivers, including kinship and foster caregivers. Other groups available as part of treatment at discretion of case worker.

Job Finding Assistance
Description:

Job seeking assistance for those with mental illness or recovering from substance abuse. Assists clients with resumes, interview skills, job retention coaching, or other assistance as necessary to help secure and keep competitive employment.

Eligibility:
Age 18+; client of any Weber Human Services mental health or substance abuse recovery program or those who are Medicaid eligible and seeking WHS services.

Mental Health Evaluation
Description:

Provides screening, diagnostic and treatment planning services for people who are experiencing acute or chronic psychiatric problems.

Eligibility:
Adults and youth eligible for Medicaid, who reside in Weber and Morgan Counties. Description:

Emergency consultation provided by licensed professionals. The agency, along with McKay-Dee Hospital jointly determine the need for psychiatric hospitalization.
