Bear River Area Agency on Aging

Area agency in support of the aging senior citizen population. Services include home delivered meals, shopping and medical transportation, caregiver counseling, caregiver and care receiver support groups, wellness programs, general health insurance information/counseling, and a long term care ombudsman program.

Description:

The Caregiver Support Program provides information, assistance, individual counseling, support groups, training, respite, and some medical equipment and/or supplies to individuals who are caring for a loved one 60 years or older. This program also provides services to grandparents who are 60 years or older and are the primary caregivers living with grandchildren 18 years or younger, who are affected by mental retardation or have developmental disabilities.

Eligibility:
Care givers and/or the care receivers must be over 60 years of age and the care giver must be actively involved in caring for the recipient.

Description:

Counselors provide Medicare beneficiaries with objective information on Medicare benefits, coverage options, supplementing Medicare, prescription drug plans, and available programs that may help pay for medications. The program is committed to helping seniors make educated decisions by providing free and unbiased Medicare insurance counseling by telephone and face to face interactions, public presentations, and media activities. The Senior Medicare Patrol (SMP) empowers and assists Medicare beneficiaries, their families, and caregivers to prevent, detect, and report health care fraud, errors, and abuse through outreach, counseling, and education.

Age 65 and older or those with questions approaching Medicare age or their families or caregivers

Description:

Advocates on behalf of residents of nursing homes, assisted living facilities, and other residential care facilities licensed by Utah Department of Health by voicing and resolving the concerns that elderly residents encounter. The Ombudsman helps residents to resolve complaints and concerns and informs residents and their families about the resident's rights.
