Eligibility:
Pregnant or breastfeeding woman, woman who has just had a baby, or child less than five years of age who meet income requirements.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

M - Th 8 am - 6 pm; F 8 am - 1 pm

Fees:
$22 for initial certificate, $10 for each additional copy. Fee waivers may be available.
