Description:

Provides accredited nursing services in the patient's home, including medication management, pain control, rehab therapy, and disease management.

Eligibility:
Homebound, physician's orders required

Description:

Delivers end of life care for comfort of patients including physical, speech, and occupational therapy, as well as medication and pain management, companion support, and bereavement services for families.
