DDI VANTAGE Early Intervention - Duchesne

Provides for children under the age of three, and their families, who have developmental delays and disabilities, by offering assessments and early intervention.

Developmental Assessment
Description:

Offers assessments to determine child's needs, minimize the effects of the developmental delay, and refer the child to appropriate services, thereby reducing the need for long-term intervention throughout the child's school years.

Eligibility:
Child must be under 3 years of age and be determined to have a developmental delay in one qualifying area or by medical diagnosis.
Service Hours:
M - F 8:30 am - 4:30 pm

Fees:
No cost for developmental evaluations and assessment.

Description:

Minimize the effects of the delay thereby reducing the need for long term intervention throughout the child's school years.

Eligibility:
Child must be under 3 years of age and be determined to have a developmental delay in one qualifying area or by medical diagnosis.

Fees:
No cost for developmental evaluations and assessment. Families may be required to pay a nominal fee based on a sliding scale as mandated by the Utah State Health Department. Some services are offered for private pay and Medicaid/CHIP are accepted.
