DDI Vantage Early Intervention - Tooele

Provides services for children under the age of 3 who have developmental delays/disabilities and their families by offering developmental assessments, early intervention for children with disabilities/ delays, and home based parenting education.

Developmental Assessment
Description:

Provides assessments for children under 3 to determine if they have a qualifying developmental disability.

Eligibility:
Children from birth to 3 years of age who have a developmental delay or disability. Family must live within Granite, Murray, Salt Lake, Tooele, or Duchesne school districts.
Service Hours:
M - F 8:30 am - 4:30 pm

Home Based Parenting Education
Description:

Parent training in the home to families with infants and toddlers with developmental delays and disabilities.

Eligibility:
Children from birth to 3 years of age who have a developmental delay or disability. Family must live within Granite, Murray, Salt Lake, Tooele, or Duchesne school districts.
Service Hours:
M - F 8:30 am - 4:30 pm

Fees:
Sliding scale fee on a case by case basis. No cost for families with TANF, WIC, Medicaid, or CHIP.

Early Intervention for Children With Disabilities/Delays
Description:

Provides services for children under the age of three who have developmental delays and disabilities and their families by offering a full range of services to meet their individual needs.

Eligibility:
Children from birth to 3 years of age who have a developmental delay or disability. Family must live within Granite, Murray, Salt Lake, Tooele, or Duchesne school districts.
Service Hours:
M - F 8:30 am - 4:30 pm
