Fourth Street Clinic

A comprehensive medical clinic, providing general counseling, general dentistry, contact lenses/glasses, mental health evaluations, navigator programs, and prescription expense assistance services to homeless individuals and families in the Salt Lake area. Related volunteer opportunities also available.

Navigator Programs
Description:

Certified Application Counselors are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP.

Eligibility:
Homeless & patients with an income less than 200% of Federal Poverty Level.
Service Hours:
M, W, Th 8 am - 6 pm; Tu 9 am - 6 pm; F 8 am - 5 pm
Intake Procedure:
Walk in.

Glasses/Contact Lenses
Description:

Provides glasses only, for homeless persons in need.

Eligibility:
Homeless & patients with an income less than 200% of Federal Poverty Level. Must have a referral from Dr. on staff.
Service Hours:
2nd Tu of each month 5 - 5:30 pm
Intake Procedure:
Call for appointment.
Fees:
Sliding fee based on income; all insurance types accepted.
Counties Served:

General Dentistry
Description:

Offers exams, x-rays, fillings, extractions, and dentures.

Eligibility:
Must be a registered medical patient of the clinic and be in good standing without a cessation of service order on file.
Service Hours:
M - Th 9 am - 6 pm
Intake Procedure:
Call to make an appointment. If no appointment, client can be placed on a no guarantee wait list.
Fees:
Sliding fee based on income, all insurance type accepted.

General Counseling Services
Description:

Counseling, crisis intervention, mental health, and substance use for homeless individuals.

Eligibility:
Homeless & patients with an income less than 200% of Federal Poverty Level.

M, W, Th 8 am - 6 pm; Tu 9 am - 6 pm; F 8 am - 5 pm
Intake Procedure:
Must have referral from Dr. on staff. Call for appointment for established patients only, walk ins welcome.
Fees:
Sliding fee based on income, all insurance types accepted.

Health/Disabilities Related Volunteer Opportunities
Description:

Volunteer opportunities related to administration, MD's, APRN's, PA's, women's health, and class assistant.

Eligibility:
Must be 18 and able to pass a background check.
Service Hours:
M, W, Th 8 am - 6 pm; Tu 9 am - 6 pm; F 8 am - 5 pm
Intake Procedure:

$37 for background check.

Prescription Expense Assistance
Description:

Established patients have access to pharmacy services on site. Naloxone available at front desk.

Eligibility:
Uninsured Homeless and patients with an income less than 200% of Federal Poverty Level.
Service Hours:
M - F 9 am - 12 noon & 1 - 5 pm
Intake Procedure:
Call for appointment, walk ins accepted for pregnancy and TB tests/reads.
Fees:
Sliding fee based on income, insurance not needed.

Mental Health Evaluation
Description:

Has limited mental health evaluations. Staffed with APRNs and LCSWs on staff that can provide mental health evaluations under their scope of practice.

Eligibility:
Homeless and patients with an income less than 200% of Federal Poverty Level.

M, W, Th 8 am - 6 pm; Tu 9 am - 6 pm; F 8 am - 5 pm
Intake Procedure:
Call for appointment for established patients (in the last two years), walk ins accepted for new patients.

Sliding fee based on income, all insurance types accepted.

Description:

A comprehensive medical clinic, providing coordinated primary care, behavioral health, limited dental, and pharmacy services to homeless individuals and families.

Eligibility:
Homeless & patients with an income less than 200% of Federal Poverty Level.
Service Hours:
M, W, Th 8 am - 6 pm; Tu 9 am - 6 pm; F 8 am - 5 pm
Intake Procedure:
Call for appointment if established patient (within the last 3 years), walk ins accepted. New patients are advised to walk in and register. Pharmacy, Dental, and Behavioral Health services are available for established patients only.
Fees:
Sliding fee based on income; all insurances types accepted.
