Description:

Health care coordination, case management, and prescription assistance for uninsured children, families, and adults including health education and specialty care coordination. Appointments can be made by any Northern Utah resident at any Midtown Clinic; locations available in Cache, Davis, Salt Lake, and Weber Counties.

Eligibility:
Prescription expense assistance is available to any Midtown Clinic patient, generally patients are under 200% of Federal Poverty Level, call for specific eligibility questions.
