Eligibility:
Those residing in Daggett, Duchesne, and Uintah counties.
Service Hours:
M - F 8 am - 5 pm
Intake Procedure:
Walk in, call, or visit website to complete documentation prior to intake scheduling). During the intake appointment (approximately 1 hour), a clinician will gather additional information through a confidential interview; then recommendations will be made. Emergencies will be directed to a crisis clinician.
Fees:
Works with most insurances; clients are encouraged to check with their provider for pre-approval, co-pays, and coverage. Sliding scale fees available based on income and number of people living off that income. No one will be denied assistance based on in
