Eligibility:
For families in Nephi, Mona, and Levan area. Application will determine eligibility.

M - Th 7:30 am - 5:30 pm; F 8 am - 12 noon
Intake Procedure:
Pick up and drop off an application between November 1st and December 5th during business hours or use drop box. No late applications will be accepted.
