Sexually Transmitted Disease Screening
Description:

Offers confidential STD testing and treatment including Rapid HIV, Hepatitis B/C, Syphilis, and Chlamydia/Gonorrhea.

Description:

Description:

Prenatal Care
Description:

The Baby Your Baby Card covers outpatient pregnancy related services such as doctor's appointments and prescriptions. It does not cover labor and delivery.

Eligibility:
Financial help is available on a temporary basis to Utah women who are pregnant and do not have the money to pay for prenatal care. Income restrictions apply. US Citizens or Green Card holders for the last 5 years may apply.

Description:

Immunizations for children, adolescents, and adults

Enrolled in Medicaid, CHIP, American Indian or Alaskan Native, or not insured.

Description:

Provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Pregnant or breastfeeding women, women who has just had a baby, or child less than five years of age who meet income requirements.

Child Passenger Safety Seat Inspections
Description:

Car seat safety check to make sure your car seat is the right size for your child and that it's properly installed in your vehicle.

Birth Certificates
Description:

Certificates for births that occurred anywhere in Utah from 1951 to the present. If the birth occurred before 1951 in Utah, contact the Department of Health and Human Services.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

Applications may be submitted in person, online, and through the mail.
Fees:
$22 for initial certificate, $10 for each additional copy; fee waivers available for those experiencing homelessness from local homeless service providers, call for information.

Description:

Certificates for deaths that occurred within Uintah, Duchesne, and Daggett County from 1990 to 2006 and deaths that occurred anywhere in Utah from August 2006 to the present.
