Provides environmental hazards information, death and birth records, STD screening, prenatal care, child passenger safety seat inspections, WIC, restauraunt and food sanitation/permits, and immunizations.

Environmental Hazards Information
Description:

The Environmental Health Division aids in the elimination of all environmental factors that lead to disease, lessen quality of life, and degrade the physical environment.

Child Passenger Safety Seat Inspections
Description:

Car seat safety check to make sure your car seat is the right size for your child and that it's properly installed in your vehicle.

Eligibility:
Vehicle and child must both be present.

Death Certificates
Description:

Certificates for deaths that occurred within Uintah, Duchesne and Daggett County from 1990 to 2006 and deaths that occurred anywhere in Utah from August 2006 to the present.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

M - F 8 am - 4:30 pm
Intake Procedure:
Applications may be submitted in person, online, and through the mail.
Fees:
$30 for initial copy; $10 for each additional copy

Birth Certificates
Description:

Certificates for births that occurred anywhere in Utah from 1931 to the present.

Eligibility:

Description:

Offers confidential STD testing and treatment. Rapid HIV, Hepatitis B/C, Syphilis, Chlamydia/Gonorrhea

Prenatal Care
Description:

The Baby Your Baby Card covers outpatient pregnancy related services such as doctor's appointments and prescriptions. It does not cover labor and delivery.

Eligibility:
Financial help is available on a temporary basis to Utah women who are pregnant and do not have the money to pay for prenatal care. Income restrictions apply. US Citizens or Green Card holders for the last 5 years may apply.

Description:

Provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Pregnant or breastfeeding woman, women who has just had a baby, or child less than five years of age who meet income requirements.

Restaurant/Food Sanitation
Description:

Regulates food service establishments including restaurants, delis, food carts, cafeterias, caterers, concession stands, and various other businesses and organizations that engage in food preparation and service. Also regulates temporary food events (booths) and temporary mass gatherings.
