Description:

Program is designed to reconnect people with their family or friends to prevent homelessness, and for people seeking shelter by identifying immediate alternative housing arrangements and, if necessary, connecting them with services and financial assistance so they can return to permanent housing.

Eligibility:
Single men and women, including individuals with partner, who are presently homeless and Family Shelter Diversion program which is for families with children under 18 who are presently or imminently homeless.

M - F 8:30 AM - 5 PM for single men, women, and partners Su - Sa 8 am - 6 pm families through the Midvale Road Home Family Shelter

UCA - Weatherization Program
Description:

Eligibility:
Applicants must be at least 18, a legal citizen or resident, the primary homeowner, and meet income requirements. Call or visit website for complete eligibility guidelines.

To qualify for crisis service, clients must be the homeowner and currently on the HEAT program. HEAT and weatherization applications must be submitted under the same household client name and be a US Citizen.

The Siemer Program is a United Way supported Collective Impact housing /case management program which serves qualifying families for children attending promise partnership community schools in South Salt Lake, Midvale, West Valley, and Kearns with financial assistance. There is homeless family outreach available for students enrolled in a school in the Salt Lake City school district.

Helps resolve conflicts and answer questions between landlords and tenants in areas such as security deposits, eviction attempts, lease issues, repairs, etc. The landlord tenant mediator is a neutral 3rd party. The landlord tenant mediator does not give rental assistance or pay back rent that is due.
