Psychiatric Case Management
Description:

Provides mental health evaluation and treatment including assessments, counseling, and psychiatry.

Eligibility:
Service Hours:
M - F 8 am - 12 Noon & 1 - 5 pm
Intake Procedure:
Call to schedule an intake appointment. Intakes take about 2 hours. Children must be accompanied by their legal guardian.
Fees:
Accepts Medicaid, Medicare, and private insurance. A Sliding scale is available for Wasatch County residents with no insurance. Does not accept PCN.

General Assessment for Substance Use Disorders
Description:

Provides substance abuse assessment and treatment. Classes and outpatient treatment available.

Eligibility:
For adults and adolescents. Accepts court appointed.

Family Life Education
Description:

Provides a 10 week program to improve communication and strengthen family relationships. A light dinner is provided each week followed by separate group work for parents and children. Parenting skills classes are also available but only for clients already participating in a service with the Family Clinic.

At least one family member needs to have Medicaid insurance

DUI Offender Programs
Description:

Provides DUI education for those who have been court ordered or self referred.

Eligibility:
For adults and adolescents. Accepts court ordered.
